Reference Letter Request Form

If you are a current or former student of mine, and need a reference letter from me for an application, please fill out the form below.

If you are applying for different schools, programmes, scholarships, internships, jobs etc., please submit a seperate form for each application.

Please contact me to ask for my permission before you fill out this form. If you submit this form without contacting me to ask for a reference letter, I cannot promise to submit a letter.

Avoid using the "Enter" key on your keyboard before completing all sections, as otherwise your answers may be submitted as incomplete. So, just add spaces (rather than paragraph breaks) if your answers are longer than a paragraph each.

Add a brief statement (no more than 3 sentences) how your current/previous studies have prepared for you in this regard - if possible make a reference to a module you studied with me.

Please list 4-5 highlights regarding your academic strengths that are relevant to this application. These must be things you can prove (and I can confirm). If I have interacted with you only as part of a large cohort, please add a note on what makes you stand out in that cohort.

If I have assessed your work before and given you written comments, please copy/paste them here. (Oxford students: If you have access to a TMS or OxCort report that I wrote about you, please copy/paste it here.)
